A wire company fined after a worker receives a fatal head injury

A wire company has been fined £140,000 after a worker was fatally injured after becoming entangled in unguarded machinery. The company pleaded guilty to breaching 2(1) of the Health and Safety at Work etc Act 1974.

A Health and Safety Executive (HSE) investigation found that the company had failed to take effective measures to prevent employees from accessing dangerous moving parts of the machine. The HSE stated that the company should have appointed a designated competent person on the site who would provide formal training rather than relying on verbal instruction.

Recognised industry safety measures could have been implemented on the machines, rather than allowing the conditions to persist.
