Advertising firm fined £400,000 after maintenance worker killed

The maintenance worker suffered fatal injuries whilst trying to repair an industrial overhead door at the company’s manufacturing facility.
The worker was attempting to re-tension the springs with an industrial wrench when the tool slipped. The spring unwound in an uncontrollably, causing the wrench to be ejected and strike him.
The investigation by the Health and Safety Executive (HSE) found the company had failed to adequately maintain the electrically operated sectional overhead doors at the site despite two previous incidents involving failed doors that had resulted in injured employees. There was no routine inspection or preventative maintenance. It was also found that the maintenance worker was allowed to undertake the work despite not being suitably trained to do so.
The company pleaded guilty to breaching Section 2(1) of the Health and Safety at Work etc. Act 1974 and Regulation 5(1) of the Provision and Use of Work Equipment Regulations 1998.
